Training Verifications
Requirements for Verification of Training Requests
All requests should come directly from the entity requesting the verification and MUST be accompanied by an Authorization for Release of Information signed by the former trainee. Trainings will not be verified via phone.

Medical School Training Verification
The Department of Radiology cannot verify training for former medical students who completed their training at UW-Madison; inquiries regarding medical school training should be directed to UW-Madison School of Medicine and Public Health Student Services (Sydney Jenkins, studentservices@med.wisc.edu ).
