1. If you would like access to the data, please contact David Harris at david.harris@wisc.edu, who will provide you further instructions.
2. Before receiving access to the data, a legal representative from your institution must review and sign a non-negotiable Database Access and Data Use Agreement (DADUA).
3. All fields in the DADUA must be completed before access to the data will be provided.
4. Once an acceptable DADUA has been received, access to the data will be granted.
