If your application involves scanning humans:

These additional materials include the Human Subjects Committee approval letter and the Human Subjects Committee approved consent form.

If your application involves scanning animals:

You must provide a Research Animal Resources Center Committee approval letter and a copy of your Animal Use Certification.
